ScriptShuttle Pharmacy Partner Service Agreement

SCRIPTSHUTTLE LLC — PHARMACY PARTNER SERVICE AGREEMENT ScriptShuttle.com · 727.219.6180 · Tampa Bay, Florida This agreement is between ScriptShuttle LLC, a Florida limited liability company ("ScriptShuttle"), and the pharmacy submitting this form ("Partner"), and is effective as of the date of electronic submission. SERVICES — ScriptShuttle will provide same-day prescription pickup and delivery to Partner's patients within the Tampa Bay service area. All pickups are scheduled between 10:00 AM and 12:00 PM Monday through Saturday. Deliveries are completed by 6:00 PM the same day. Delivery confirmation is sent to Partner upon completion of each delivery. PRICING — Partner selects one plan at enrollment. Basic is $10.00 per delivery with no monthly fee and no minimum. Starter is $299 per month covering up to 30 deliveries with additional deliveries at $9.50 each. Growth is $599 per month covering up to 75 deliveries with cold chain included and additional deliveries at $8.50 each. Partner is $999 per month covering up to 150 deliveries with cold chain and priority routing included and additional deliveries at $7.50 each. Invoices are issued weekly or monthly per Partner preference and due within 7 days of receipt. Outstanding balances at cancellation remain due within 15 days. HIPAA COMPLIANCE — Both parties agree to execute a separate Business Associate Agreement prior to ScriptShuttle handling any patient prescription or protected health information. ScriptShuttle maintains HIPAA compliance and general liability insurance of no less than $1,000,000 throughout the term of this agreement. CANCELLATION — Either party may cancel with 7 days written notice by text or email. No cancellation fee. Subscription plans receive a prorated refund for unused days in the current billing period. All outstanding delivery fees remain due within 15 days of cancellation. LIABILITY — ScriptShuttle's liability for any lost or damaged prescription is limited to $500 per incident. ScriptShuttle is not liable for delays caused by incorrect patient information provided by Partner, patient refusal of delivery, or circumstances outside ScriptShuttle's reasonable control. GOVERNING LAW — This agreement is governed by the laws of the State of Florida. Any disputes shall be resolved in Hillsborough County Florida. ELECTRONIC SIGNATURE — By submitting this form the Partner acknowledges they have read and understood this agreement and that their typed name constitutes a valid electronic signature under the federal Electronic Signatures in Global and National Commerce Act and Florida Statute 668.50.
